Loan Cancellations/Reductions

Loan Cancellation/Reduction Request

View Description

This form may be used to cancel or reduce Stafford, Perkins, and PLUS loans. NOTE: The Office of the Bursar normally will not return funds for private loans after the loan has credited to a student´s bursar account. Cancellations of private loans will only be made if the student is not enrolled. Students should contact the Office of Student Financial Assistance directly if they are interested in canceling or reducing an undisbursed portion of a private loan.

This electronic form is for reporting scholarships, grants, assistantships, fee remissions, veterans' benefits, or other assistance you will receive during the upcoming award period which are NOT listed on your Financial Aid Notification.

Applications

FAFSA (Free Application for Federal Student Aid)

View Description

The FAFSA is the application required for most types of need-based aid. The FAFSA must be completed in order for a student to be considered for federal and state grants, federal student loans (including Stafford and Perkins), and Federal Work-Study. Students should complete the 2007-2008 FAFSA for aid for the 2007-2008 academic year, and 2008 summer. Students should complete the 2008-2009 FAFSA for the upcoming 2008-2009 academic year.

Federal SMART Grant Recipient, Coursework Verification

View Description

Federal SMART grant recipients will use this form to verify that they are enrolled in at least one course which meets the specific requirements of their major.

SSACI Minority Teacher/ Special Education Services Scholarship

View Description

The Minority Teacher/Special Education Services Scholarship is administered by the State Student Assistance Commission of Indiana; a student must be an Indiana resident to be eligible. Students must also complete the appropriate academic year´s FAFSA. Applications must be submitted to the Office of Student Financial Assistance. The priority deadline is July 1 (prior to the start of each academic year).

Federal Parent PLUS Loan

The Federal Parent PLUS Loan may be borrowed by a biological or adoptive parent, or a step-parent, on behalf of a dependent, undergraduate student. The loan is subject to credit approval.

If a parent is denied the Federal Parent PLUS Loan, the student may be eligible to borrow an additional Federal Stafford Unsubsidized Loan beyond the regular dependent borrowing limit.

Child of Certain Veterans and Public Safety Officers Supplemental Grant Program

View Description

This program provides tuition assistance for eligible children of disabled Indiana veterans, children of POW/MIA from the Vietnam War, eligible children/spouses of certain members of the Indiana National Guard killed while serving on active duty, and eligible children/spouses of certain Indiana public safety officers killed in the line of duty. In addition, applicants must also file the appropriate academic year´s FAFSA and pay Indiana resident tuition/fees.

PPST/Praxis II (NTE) Fee Waiver Release Statement

This form is used to verify eligibility for the PPST/Praxis II (NTE) fee waiver, and to secure permission from the student to release confidential financial aid information.

SSACI Nursing Scholarship

IU Campus Consortium Application

IU Bloomington students who wish to receive financial aid while attending another IU campus must complete and be approved for a consortium before their IU Bloomington financial aid can be released. This form details information that must be submitted (through e-mail).

Appeals

Special Conditions Forms

View Description

Special Conditions Forms may be used to increase a student´s cost of attendance due to special expenses, or reduce a student´s Expected Family Contribution due to a loss of income or other benefits. Changes made through a Special Conditions Form may or may not increase a student's aid eligibility.

Satisfactory Academic Progress Appeal

View Description

Students who have been denied aid due to unsatisfactory academic progress must appeal and be approved in order to receive financial aid (including private and PLUS loans).

Dependency Appeal

View Description

Dependent students seeking independent status as a result of an irretrievable breakdown or other extenuating circumstances must appeal. This form details the documentation that must be provided in order to appeal.

Previous Dependency Appeal Confirmation Form

View Description

Students who were granted independent status for the previous year must complete this form, verifying that they should continue to be considered an independent student. Also included on this form is information regarding the filing of the FAFSA. Students who need to complete this confirmation process must call our office and make an appointment to review their completed information. (Previous dependency override recipients will be sent an email in January with this form. It can also be obtained at our office.)

Independent Student: Orphan or Ward/Dependent of the Court Worksheet

View Description

Undergraduate students who are independent by virtue of being an orphan or dependent of the court must use this worksheet to verify their situation; students who are orphans or dependents of the court (until age 18) may then be considered independent. .

Independent Student Dependent Support Form

View Description

Undergraduate students who would otherwise be considered dependent, but may be independent by virtue of supporting a dependent, must verify their status each year using this form.
