Membership Application

To become a member you must do the following:

  1. 1)

    Download and fill out the IC-EMS membership form.

  2. 2)

    Print the completed form and sign page 3

  3. 3)

    Photocopy the front and back of any current CPR/AED, First Aid, or EMT-B certifications you have.

  4. 4)

    Place the photocopies, your membership dues ($40 for the entire school year, $20 for a single semester), and your completed membership form in an envelope with your name clearly written on the front.

  5. 5)

    Drop off (DO NOT MAIL) all items to our office: Indiana Memorial Union, Student Activities Tower, Suite 672.

  6. 6)

    Email a scanned copy (front and back) of your certifications to icems@indiana.edu.

*NOTE: In order for you to volunteer with IC-EMS, your certifications must be on file.

Application Forms

All membership applications due Friday, 2 October 2015.

If you want to work Dance Marathon or Little 500 events, your membership application must be submitted.
