Outreach Activity Tracking Form

African Studies faculty and students engage in various outreach activities in the course of the academic year. The African Studies Program needs to track such activities for the purpose of Title VI reporting. In order to facilitate our Title VI reporting process, we would like to ask our faculty and students to use this electronic form to record their outreach activities.
